How do you set up a scenario ?

Many do BV Balancing some by tonnage . Some by what resources should be defending a target vs what you or a client can be brought to bear even if it means a protracted conflict waging a war of attrition before making a decisive push for victory risking the enemy doing the same before you take too much out of there forces. What guidelines if any does your campaign use.

I have run a long on going campaign mostly so my OPFOR has whatever I think will challenge the players.  They annoyingly enough typically roll better despite my best efforts.
