This was part of a free PDF chart collection made for Tactical Operations, available shortly after the book's release on the BT website.  After that iteration of the website died, it never was uploaded again.

I believe that CGL has (Or had) plans to re-release that PDF and similar ones for the other 4 early core books once they get around to updating the branding, and probably changing some things in the TacOps one (I believe that the list your after may have changed all the errata released over the years, at the very least page numbers will have changed)
