The first sentence seems unclear. Some people interpret it to mean a force with 12 units or less may have one additional SPA. While other people interpret it to mean a force must have at least 12 units or more to have one additional SPA.

Which is the correct interpretation?

The second sentence mentions "total SPA cost" as a limiting factor.

Is this saying the additional SPA must be paid for with PV, or is it free?

1-12 units: 1 with SPAs
13-24 units: 2 with SPAs
etc

For the total SPA cost, there is no PV cost for SPAs.  It is referring to the limits on p92 "assigning special pilot abilities".  Where a Veteran (skill 3) unit is limited to 2 SPAs with an SPA Cost total of 4 or less.
