Vindicator artwork - Anyone have an artists name for this?

Does anyone have more information about this piece of art? An artist's name would be cool.

Could be Laubenstein's work, but that's just a guess

The chest plate has a name and date but I can't read it.

Chest and left arm read "Chrystal's Crusher", while text just past the end of the right arm barrel looks like RS 04 to me.  What book was this originally from again?

I honestly don't remember this being from one of the books. The first time I saw the artwork, it accompanied a poem written from the perspective of the mech after having been cast off and abandoned by its pilot.
