Usage of eponymous "last names"

I've seen a few Clanners referred to with their Clan name as a sort of last name, rather than a Blood Name. 

Under what circumstances is this done?

When is someone known as, say, Kim Nova Cat, as opposed to just "Kim"?  Is it just something IS people do because they're obsessed with last names?

signifies that the person is not born of the clan, but rather was adopted into it.

I don't think that's right. All of the Dark Age pilot dossiers referred to Warriors without Bloodnames as Kim Wolf or Kim Nova Cat or whatever. The Mystic Caste warriors also used "Nova Cat" as a surname.

The only time it is used is when a warrior is Freeborn but becomes a part of the Warrior caste- hence, Jaime and Joshua Wolf, Elson Novacat, and so on.

Phelan used the name Wolf because he was an adoptee into the warrior caste, not because he didn't have a Bloodname. We didn't see Vlad, for instance, being called Vlad Wolf before winning his Bloodname.

It is right. But by the Dark Age, it's clear that practices have changed somewhat. Some Clans are a lot more accepting of freeborn warriors winning Bloodnames, for instance.

Phelan was Freeborn and adopted into the Warrior caste, though.

Trueborns are [First Name] of the [Bloodhouse they are part of]; Vladimir of the Wards, Ranna of the Kerenskys, etc.
